Fatality Prevention Mindset

EONZ have made the following Coroner’s reports available to assist participants of the Fatality Prevention Workshops expand their knowledge base. The workshops discuss the principles of Fatality Prevention and the benefits that can be gained by being familiar with cases so that educators can learn from the past – and in so doing avoid repeating mistakes from the past. The cases made available here supplement the workshops and should not be seen as the sole source of developing a Fatality Prevention Mindset.

Please note that these Coroner’s reports mention specific people and circumstances which have an ongoing impact on families and communities. Please be mindful that these cases contain material that some may find disturbing. If you choose to access these cases please do so with an attitude of understanding and compassion for all involved and use them to build your understanding so that you can look to the future with vigilance.
